Appointments 

Fact: Our office runs on time. So as not to infringe on other patients' scheduled visit times, please always arrive ten minutes before your scheduled appointment time. A parent or legal guardian must accompany anyone under age 18 for their appointment. Initial visits last approximately 60 to 90 minutes. Follow-up sessions run about 60 minutes.

 

 

 

What to Bring to Your Appointment

  • Your written referral or consult form from a physician, nurse practitioner, physician's assistant podiatrist or dentist

  • Your current insurance cards, information, and ID card(s)

  • Co-pay and deductible payments

  • A list of all the medications (names and dosages) you are currently taking

  • Comfortable, loose-fitting clothing, sneakers, sweatpants, shorts, T-shirt

 

 

 

 24 Hour Cancellation Notice Policy

Our office sets aside time for you to receive the most attentive service and quality care possible. We are a team working together to achieve a common goal, that is, achieving the goals set for your improvement on your initial visit. Your therapist is responsible for your care and you are responsible for compliance with attandance to your scheduled therapy sessions and performing your assigned home exercises. Patients who do not arrive for a scheduled appointment or cancel an appointment without 24 hours notice are personally responsible to pay a fee before continuing therapy.
